Once a patient is referred to us, we assist with every aspect of their needs. We assemble a dedicated nursing team with the mission of helping the client achieve his or her goals. We assist individuals and families with customized programs of care that improve outcomes and increase independence.
Brain and Spinal Cord Injury
Since each patient is unique, our clinical training and programs of care are based on best practices of home health nursing services related to a myriad of injuries.
Achieve Services provides professional nursing services for both Brain Injury (ABI) and Spinal Cord Injury (SCI) clients. Our programs are models in their respective fields.
We work to meet the needs of the following:
1.Full-assistance for clients with Brain Injuries and/or Spinal Cord Injuries who will benefit from a coordinated 24/7 team approach
2. Work-related injured patients who benefit from working with nursing professionals to achieve identified plan goals
3. Patients who need a Catastrophic Injury program of rehabilitation for multiple trauma, ventilator or would care
4. Patients who have Short Term nursing needs requiring Certified Nursing Assistance
