Since each client is unique, our training is based on best practices of home health nursing services related to a myriad of injuries.

Achieve Services provides professional nursing services for both Brain Injury and Spinal Cord Injury clients. Our programs are models in their respective fields.

Our caregivers are trained using our best known techniques and methods in providing professional nursing services for catastrophically injured clients.

Our process is simple. Once a client is referred to us, We assess the client's needs. We assemble a professional nursing team with the mission to help the client reach his or her goals, and assist with the program of care to help the client maximize improvement.

Our Team

When additional services are needed to facilitate client recovery, promote independence or maintenance after inpatient or outpatient rehabilitation services, Achieve Services provides reliable and qualified nurses.

Our team consists of the following:

Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants. Our seasoned professionals are trained to manage patient care. Our nurses are responsible for overall implementation of care plan and coordination and maintenance of the
patient care needs.

Catastrophic Injury

Catastrophic injury normally covers very severe injuries, often resulting in paralysis. At the lower end of the scale are amputations or fully paralyzed, and at the higher end paraplegia, tetraplegia, quadriplegia or even a permanent vegetative state. Cases may be further complicated by fractures to the spine or even multiple fractures. These patients invariably require planning for the long-term, usually the rest of the client's life. While the road to recovery is difficult it is imperative to focus on recovery and trying to get the client*s life back. Someone else needs to be out there fighting in their corner.

Our professional experience and expertise with catastrophic injuries whether they are amputations, spinal injuries, head injuries or serious orthopedic/neurological injuries focuses on life skills training and quality of life.

We are absolutely committed to helping with all the needs to the best of our abilities of our clients and their families. We will not falsely raise hopes, nor will we set unrealistically low targets but work with all the clinical team members and the orders of the attending physicians to implement the long term plan in the home. As long as it is to the benefit of our clients, we work daily to that each person gets the best home nursing services with the goal of returning to a meaningful life and/or work.

Brain Injury

Achieve Services provides a continuum of post-acute home nursing rehabilitation programs to persons with acquired and traumatic brain injury. Our programs improve quality of life and maximize independent functioning in daily living, work, and leisure in the home and community environment.

Persons with acquired or traumatic brain injury often experience changes in physical, cognitive and emotional functioning. Achieve Services believes each person should have an individualized and integrated treatment plan that respects his or her right to make informed choices, and to fully participate in all aspects of treatment planning and implementation. Clients, with family involvement and our qualified staff, develop individualized plans of care for their respective family situations.
